6. Image Waiver (new families only)

7. Consent to Medical Treatment (new families only)

8. Medical Records Release

9. Parental Consents, including “CONSENT TO TRAVEL BY MINOR UNACCOMPANIED BY PARENT(S) OR GUARDIAN(S)", and “AUTHORIZATION FOR THINK GLOBAL SCHOOL TO ACT IN THE PLACE OF PARENTS/GUARDIANS" (must be paper copy notarized in triplicate)

10. Medical Information Form

11. Physical Examination and TB Screening Form (to be completed by health care provider)

Documents to Send to TGS

1. Copy of student's passport(s)

2. Copy of student's birth certificate or adoption certificate, with English translation if applicable (new families only)

3. Copy of the student's final, signed, and official transcript for the 2023-2024 school year, with English translation if applicable (new families only)

4. Supporting Documents for sole custody parents (only if requested by TGS)
